PROJECT REQUEST

INSTRUCTIONS:

1. Please fill out the entire form giving as much detail as possible.

2. Only projects that can be completed during the event will be considered.

3. Property improvement projects will only be considered if the property is owner occupied.

4. Projects requiring the use of heavy machinery will not be considered.

5. You will be notified if your project is selected or denied.

6. Feel free to attach additional information as needed. (pictures may be useful)
